BODY CAVITIES:

1. No adhesions or abnormal collections of fluid are in any of the body cavities.

2. All body organs are present in normal anatomical positions.

HEAD (CENTRAL NERVOUS SYSTEM):

1. The brain weighs _______ grams.

2. The dura mater and falx cerebri are intact.

3. The leptomeninges are thin and delicate.

4. The cerebral hemispheres are symmetrical.

5. The structures at the base of the brain, including cranial nerves and blood vessels, are intact and free of abnormality.

6. Sections through the cerebral hemispheres reveal no lesions within the cortex, subcortical white matter, or deep parenchyma of either hemisphere.

7. The basal ganglia, thalami, and hippocampi are unremarkable.

9. Sections through the brainstem and cerebellum are unremarkable (state if brain was fixed in formalin and examined with neuropathologist).

NECK:

1. Examination of the soft tissues of the neck, including strap muscles, thyroid gland, and large vessels, reveals no abnormalities.

2. The hyoid bone and larynx are intact.

3. The lingual mucosa is intact; the underlying firm red-brown musculature is devoid of hemorrhage.

CARDIOVASCULAR SYSTEM:

1. The heart weighs_______grams.

2. The pericardial and epicardial surfaces are smooth, glistening and unremarkable; the pericardial sac is free of significant fluid or adhesions.

3. The coronary arteries arise normally, follow the usual distribution and are patent, without evidence of significant atherosclerosis or thrombosis.

4. The chambers and valves bear the usual size-position relationships and are unremarkable. The valves have the following circumferences: tricuspid valve = ____ cm, pulmonic valve = cm, mitral valve = cm, and aortic valve = cm.

5. The myocardium is dark red-brown, firm, and unremarkable; the atrial and ventricular septa are intact. The left ventricular thickness = cm, right ventricular thickness = cm, and the interventricular septum = cm.

6. The aorta and its major branches arise normally, follow the usual course and are patent, free of significant atherosclerosis and other abnormality.

7. The venae cavae and their major tributaries return to the heart in the usual distribution and are free of thrombi.

RESPIRATORY SYSTEM:

1. The right and left lungs weigh ____ and ___ grams, respectively.

2. The upper airway is clear of debris and foreign material; the mucosal surfaces are smooth, yellow-tan and unremarkable.

3. The pleural surfaces are smooth, glistening and unremarkable. Lobar divisions are of the usual configuration.

4. The pulmonary parenchyma is dark red-purple, exuding slight to moderate amounts of blood and frothy fluid; no focal lesions are noted.

5. The pulmonary arteries are normally developed, patent, and without thrombus or embolus.

LIVER AND BILIARY SYSTEM:

2. The hepatic capsule is smooth, glistening and intact, covering red-brown parenchyma, with no focal lesions noted.
